ADMISSION

Admission may be done through Onsite (Face-to-Face) or Online transactions

For Onsite Admission:

Our office hours are from 7:30 AM to 4:00 PM, Monday through Friday.

For Online Admission: Online Admission is available from 7:00 AM to 9:30 PM.

  • Payments can be made via Bank Transfer or Gcash.
  • Please send a screenshot of the proof of payment to our Facebook page (www.facebook.com/stlcqc1981) for verification and to ensure it is properly credited to the learner's account.

ADMISSION REQUIREMENTS

  •  Accomplished Online Admission Form https://stlc.schoolmate.pro/online_admission
  •  Original copy of the Philippine Statistics Authority (PSA) Birth Certificate
  •  Report Card (Form 138/SF9) with indicated Learner’s Reference Number (LRN)
  •  Certificate of Completion from the previously attended school in failure to present the Report Card (Form 138/SF9)
  •  Certificate of Good Moral Character
  •  Two (2) recent passport-sized ID pictures
  • Student Permanent Record (SF10/Form 137) For Grades 2 to 10.
  • Early Childhood Care and Development Checklist (ECCD) For Grade 1

 

AGE REQUIREMENTS FOR PRESCHOOL LEARNERS:

Pre-Kindergarden : Learner must be at least 3 years of age

Kindergarten 1 : Learner must be at least 4 years of age

Kindergarten 2 : Learner must be 5 years old by the cutoff date and extension period set by the Department of Education for the upcoming academic year.

 

  • Transferees from public and private schools who fails to submit the SF 9 (formerly Form 138) upon enrollment shall only be “temporarily enrolled” but must submit required documents within one month from the official date of opening of classes of the current school year.
  • They shall be required to submit a notarized Affidavit of Undertaking signed by the parent/guardian to be temporarily enrolled.
  • For temporarily enrolled students, the receiving school shall issue a Temporary Progress Report Card signed by the Class Adviser for the parents/guardians to monitor the progress of his/her child. This form is INADMISSIBLE FOR TRANSFER AND ENROLLMENT PURPOSES and is only issued for progress monitoring.
  • Upon complete submission of the documentary requirements, the student shall be tagged as officially enrolled in the LIS (Learners Information System). Otherwise, the student retains the status of TEMPORARILY ENROLLED, CANNOT BE OFFICIALLY PROMOTED to a higher grade level, and will not be recognized should he/she attain the qualifying average and other criteria for academic honors, and the receiving school SHALL NOT RELEASE OFFICIAL DOCUMENTS such as the SF 9, SF 10, Certificate of Completion, Diploma, etc.
  • Transferees must submit their Permanent Records/Form 137 before the end of the first grading period of the current school year. For transferred-in students during the school year, transfer of documents shall be secured thirty (30) days from the first day of school attendance.
